Step 2: We will notify you via email: Whether your child/ward can be offered a Student Care Centre (SCC) place in the school. How to register with the designated Student Care Centre (SCC) if they are offered a place. Indication of Interest Form To register your child/ward for Commit Learning SchoolHouse (CLS) Student Care Centre (SCC), please indicate your interest by filling up this online form. Important: An indication of interest is not considered registration. It is also not first come, first served. Admission into the student care centre (SCC) is not guaranteed due to limited vacancies. Instructions: Please complete *one (1) Interest Application Form for each child/ward.* Please prepare the following information before you proceed with the indication of interest: Child's/Ward's information Main applicant's information Information of spouse of main applicant (If applicable) Household information i.e. Total gross household income (GHI) Email (Please note that this email will be used for future correspondence) Singapore mobile number Please ensure that all information provided is accurate and up-to-date. If you require more time to complete the indication of interest form, click on ‘Save as Draft’. System will save only the last saved version as a draft. You will be able to modify and save/submit your indication of interest form on a later date. A draft indication of interest form link will be sent to the email address provided within this indication of interest form. Please click on the link provided to complete your indication of interest form. Please note that a draft indication of interest form will NOT be processed. You must press the “Submit” button to submit your indication of interest form. If you have successfully submitted your online indication of interest form, an acknowledgement email will be sent to the email address provided within this indication of interest form. If you do not receive the acknowledgement email, you should check the junk/spam email folder. Otherwise, you can contact us at admission_cls_phpps@commitlearning.com.sg We will notify parents/guardians who have indicated their interest via the email address provided on whether their child/ward can be offered a place in the Student Care Centre (SCC). For child/ward offered a place, the parent will be informed via the email address provided of how to register with the designated Student Care Centre (SCC).

If I have furnished false information or intentionally omitted to furnish information, Commit Learning SchoolHouse (CLS) reserves the right to require my child/ward to give up the Student Care Centre (SCC) place allocated even if my child/ward has already started attending the Student Care Centre (SCC) in the school. I will inform Commit Learning SchoolHouse (CLS) if, after the submission of this form, any updates in the personal data and other information relating to my child/ward which I have not declared in this form. Commit Learning SchoolHouse (CLS) may collect, use and disclose personal data and other information relating to my child/ward, as provided in this online indication of interest form, for the purposes in accordance with the Personal Data Protection Act 2012: the processing of this application form; the administration of enrolment related matters. Commit Learning SchoolHouse (CLS) sharing personal data and other information relating to my child/ward with the primary school for the purposes of planning the learning and development support for my child/ward. Should the other parent or guardian have any basis to contest this arrangement in the future, I undertake to resolve the issue with him/her.
